Reports
A report that answers the questions it was asked
Templates per specialty and report type keep the structure. IMECore drafts from the records and the chronology. A person signs the opinion.
The problem
The report is rebuilt each time, and the questions fall through
The examiner writes in Word from a skeleton saved on their desktop. Your questions sit in a covering letter that is not in the document. Review happens in email with tracked changes and no version control.
- The template changes between examiners and between reports.
- One of your questions goes unanswered and no one notices.
- Findings cite the chart without a page number.
- Version three is called final_v2 and nobody knows what changed.
- The opinion arrives unsigned or as a scanned image.
What changes
What IMECore does about it
A template that matches the referral
Report type and specialty set the headings, prompts, and the exhibit list. The questions you asked sit beside the draft.
Citations to the page
Point a finding at the record it came from. IMECore generates the citation in a consistent style with record and page number.
Drafted from what is on the case
The draft pulls from the ordered records, the chronology, and retrieval hits. AI drafts the summary and checks coverage. The opinion stays with the examiner.
QA gates before delivery
Required questions answered, required fields present, and consistency checks are gates. The case does not advance until they pass.
Versioned and approved
Reports carry version, status, and approved by. Draft, QA, approved, and delivered are explicit, with an audit trail on each transition.
Built for disclosure
Each document on the case carries its source and when the examiner received it. Export the exhibit index the report relies on.
How it runs
Four steps, start to finish
- 1
Open the template
Create the report on the case. The correct template loads with headings and prompts for the specialty and report type.
- 2
Draft from the case
Import the chronology and the citations. Let IMECore draft the summary and check which questions still need an answer.
- 3
Review and gate
A named reviewer checks the draft against the questions and the required fields. Comments and changes stay on the case.
- 4
Sign and deliver
The examiner signs, the reviewer approves, and the report is released. The delivered version is retained with its audit record.

FAQ
Questions people ask
Does AI write the medical opinion?
No. AI sorts records, drafts the chronology and the summary, and checks that each question has an answer. The examiner writes and signs the opinion.
What replaces the Word skeleton?
A report template managed in IMECore per specialty and report type, with the instructing questions visible beside the draft.
How do you know a question was missed?
QA gates require an answer for each question on the case. The report cannot move to approved until the gate passes or a reviewer overrides with a reason.
Can we edit in Word and bring it back?
Yes. Export the skeleton, edit in Word, and upload the finished report. Citations and version history still attach to the case.
Who approves the report?
The examiner signs the opinion and a named reviewer approves the release. Both approvals are stored with the report version.
