## How to use it

1. **Remove names first.** Strip identifiers. Paste only the form language.
2. **Read the grade and the sentence length.** The Flesch-Kincaid grade is computed locally from words, sentences, and syllables.
3. **Check the seven clauses.** Purpose, who receives the report, what is collected, not treatment, withdrawal, retention, and contact.
4. **Copy the report.** Use it to mark up the form with counsel, not as a certificate.

## A form the person cannot read is not consent

IME consent forms are often written for the file, not for the person signing. Long sentences bury the fact that this is not treatment. They bury who gets the report. They bury how long the file is kept. A reading grade does not make the form lawful. It tells you the person may not understand it. If the longest sentence runs past forty words, cut it.

## The seven clauses a Canadian IME consent should carry

State the purpose. Name the class of people who will receive the report. Say what you collect. Say this is not treatment and that no doctor-patient relationship is created. Say the person can withdraw or refuse, and what happens then. Say how long the file is kept. Give a contact for questions before they sign. If a clause is missing, the checker flags it. Keyword hits are not a legal review.
